I authorize Chelsea First United Methodist Church (Chelsea First) to use photos, videos, and other images of my child for whom I have legal guardianship, which may be used for promotional materials or posted on the Chelsea First website, social media pages, and newsletters. Such images will not be sold to other parties.

According to Chelsea First's Digital Communication Policy, no identifying information, including names, will be posted alongside a child's image.

Communication by Social Media, e-mail, and Texting Waiver

I authorize Chelsea First staff and/or volunteers to communicate with my child for whom I have legal guardianship by methods of social media, email, and/or testing. Before texting or emailing, adults shall verify that parents have signed a registration form and authorized communication via social media, email and/or testing on that form. All texting/emailing shall be in a group format. One-on-one texting/emailing with youth is not allowed. Parents and/or the staff leader must be copied on all texts and emails with youth recipients.
